PARK HOMES

Hillsboro, KS

Knowing that the traditional approach to skilled nursing was failing, Park Homes embarked on the construction of small stand-alone SNF houses for their residents and converting the existing SNF structure to assisted living and resident amenities. The plan provided two distinct and secure courtyards for residents and visitors. One passive highly landscaped with a kitchen garden and one active where residents and grandchildren could play.

OAKWOOD UNIVERSITY VILLAGE WEST

Madison, WI

​

This 60-resident skilled nursing project was a replacement for a 1960's vintage medical model nursing home on the Oakwood campus. The approach was to concentrate the nursing on sub-acute rehabilitation for campus residents and included a large PT/OT gym and pharmacy. The 3- and 4-story building contained six household 10 residents each along with household dining, kitchen and living rooms. Each resident room was designed to have both a sleeping and living area where residents could socialize with visitors and family members.

JOHN R. MOSES VETERANS HOME

King, WI

Located on a large Veterans campus in central Wisconsin, this replacement skilled nursing structure serves 192 Veterans and replaces aging buildings elsewhere on campus. The facility has three stories of private resident rooms contained in 16 households of 12 residents each. The central core of the building provides services that are distinctly separated from public access but easily accessible to staff.

LAFAYETTE MANOR

Darlington, WI

This county operated skilled nursing and assisted living replacement building provides all residents with private rooms and multiple large and small social areas enabling them to maintain relationships developed within the community. The household design provides for either 12 or 13 residents in each household and each resident room has distinct living and sleeping areas as well as abundant natural lighting.

GOUVERNEUR HEALTHCARE SERVICES

New York, NY

Originally constructed in the 1960's, this ambulatory care center operated by the NYC Health+Hospital Corporation Crepidoma Consulting provided layout and design services for the needed  modernization of their senior skilled nursing care. Located on a tight site, the upper levels of the existing building were refurbished into four households of 10 residents each per floor and included a small addition to comply with the resident yield necessary and add to the architectural façade. The existing column spacing required a unique approach to resident room design. Each household had its own living room, dining room and household kitchen and there was a central social area where residents from all households could gather.

KHOO TECK PUAT ELDERLY HOSPITAL

As a part of a major new acute care project, KTPH con-structed a senior "hospital" aimed at long-term care for the elderly. Crepidoma provided consulting services in the layout of this portion of the project.

SIENA ON THE LAKE

Racine, WI

​

As part of the master plan for converting the Racine Dominican Sisters' convent to a life plan community, skilled nursing households were included as well as a household specifically for skilled nursing sub-acute rehabilitation with a PT/OT gym located in a portion of the converted existing convent.

SIOUX CENTER NURSING

Sioux Center, IA

This replacement nursing project operated by the acute care provider in the rural Iowa community included five households with 14 residents in each household. The center of each household included living, dining and kitchen space as well as smaller, more intimate social spaces. The main services and offices were located near the household entries and resident/visitor amenities completed the central core. 

NORTH LIBERTY SENIOR CARE

North Liberty, IA

The concept for this senior community included skilled nursing, assisted living and memory support assisted living along with a small 10-resident skilled nursing rehabilitation household and large PT/OT gym that would also accommodate out-patient rehabilitation.

UTE ELDERLY VILLAGE

Duchesne, UT

​

Located on the Unitah-Ouray tribal reservation in eastern Utah, this elderly village provided skilled nursing, assisted living and a renal dialysis clinic. The skilled nursing household design respected the Ute culture particularly as it pertained to end-of-life, while the assisted living was designed to emulate the encompassing arms of a bear and provided a courtyard area where the annual Ute bear ceremony could take place.

SALEM LUTHERAN HOME

Elkhorn, IA

Located in a Dutch heritage rural Iowa community, this skilled nursing addition provided private resident rooms each with a secured patio where residents and visitor could take advantage of the outdoors in good weather. It also included one of the first dementia specific skilled nursing household in the state.
